This social worker serves as a valued member of the interdisciplinary genetics team, providing comprehensive, patient-centered care to pediatric and adult patients with rare and complex genetic and metabolic disorders, which includes conducting psychosocial assessments that address factors impacting patient and family well-being, treatment adherence, and health outcomes; delivering individualized clinical interventions that include supportive counseling, adjustment-to-diagnosis, care coordination, and crisis intervention; adhering to ethical, organizational and professional standards; and maintaining timely, accurate, and comprehensive documentation in the electronic health record to support care coordination, interdisciplinary collaboration, and continuous quality improvement. Provides developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license. Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health. Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services. Participates in the development and delivery of education, research and continuous quality improvement. Education Requirement: MSW, required. Licensure Requirement: LSW, required. LISW, preferred. Experience: 2 years clinical social work experience, preferred. Physical Requirements include occasional exposure to biohazard waste, blood and bodily fluids, driving motor vehicles, lifting/carrying various weights, loud noises, patient equipment, pushing/pulling various weights, and working outdoors. Frequent activities include bending/twisting, climbing stairs/ladder, flexing/extending neck, hand use, reaching above shoulder, repetitive hand/arm use, sitting, squatting/kneeling, standing, and walking. Continuous requirements include audible speech, communicable diseases/pathogens, computer skills, decision making, hearing acuity, interpreting data, peripheral vision, problem solving, and seeing far/near.
